Is an NHS Prescription Prepayment Certificate Worth It?

If you live in England and take regular prescribed medication, there is a decent chance you are paying more than you need to. The Prescription Prepayment Certificate exists to fix that, but only if you actually work out whether your usage crosses the break even point. Here is the maths for 2026.

The Standard Charge

Each individual prescription item in England costs £9.90 in 2026/27, frozen at last year's level. That is per item, not per prescription form, so if a single prescription lists three different medicines, that is three charges.

What a PPC Costs

A Prescription Prepayment Certificate lets you pay a flat fee upfront and then collect as many prescription items as you need for the certificate's duration, at no extra cost per item. For 2026/27, a three month certificate costs £32.05 and a twelve month certificate costs £114.50. There is also a dedicated HRT PPC at £19.80, valid for 12 months, covering specified hormone replacement therapy medicines only.

The Break Even Point

• A three month PPC pays for itself if you need more than 3 items across those three months
• A twelve month PPC pays for itself if you need more than 11 items across the year

If you are on even one regular repeat medication collected monthly, that is already 12 items a year against a threshold of 11, so the twelve month certificate is almost always worth it. If you take two or more repeat medications, the saving becomes significant, potentially over £100 a year.

Who Does Not Need One

If you are already exempt from prescription charges, a PPC adds nothing. Common exemptions include being under 16, being 16 to 18 and in full time education, being pregnant or having had a baby in the last 12 months with a valid maternity exemption certificate, or holding a medical exemption certificate for certain long term conditions. Check the exemption list before buying a PPC you do not need.

How to Get One

You can apply online through the NHS Business Services Authority, by phone, or by post, and pay by direct debit in instalments for the 12 month version rather than a single lump sum if that suits your budget better. Once approved, you show your PPC number at the pharmacy instead of paying per item.
